means that you are using FOP 0.92beta and not FOP 0.20.5!
You have installed the plug-in for a version of FOP[*] which is not
supported by the DITA Open Toolkit (not by XXE, not by embedded ant, but
by DITA Open Toolkit 1.2.2).
This was displayed by the "Options|Install Add-ons" dialog box:

This configuration comes with its own copy of the DITA Open Toolkit
1.2.2. Therefore there is no need to separately install it if you want
to convert your DITA topics and maps to HTML, PDF, etc. Just note that
generating PDF requires installing the "Apache FOP XSL-FO processor
plug-in" add-on, version 0.20.5 (not version 0.92beta). Simply use
Options|Install Add-ons one more time to do this.

Please uninstall the add-on called "Apache FOP 1.x XSL-FO processor
plug-in" and install instead the add-on called "Apache FOP XSL-FO
processor plug-in".

PS: XXE add-on manager v3.5 will have requires/excludes clauses which
will make all these problems disappear.
