My guess is that your DITA documents have references to external HTML
files and that these references have not been marked as being HTML (i.e.
have not been marked as *not* being DITA).
Example:

href="http://dita-ot.sourceforge.net/";>DITA Open Toolkit</xref>
---
Notice that I've added attribute format="html", otherwise the DITA Open
Toolkit thinks "http://dita-ot.sourceforge.net/"; is a DITA file.
Please check not only xref elements, but also any element which is a
reference (e.g. map/topicref).
If I'm right, you should have exactly the same problem if you run the
DITA Open Toolkit using its "ant" scripts from the command line.
