The IP address that the trap/syslog comes from needs to match one of the IP addresses of the device's interfaces (on OS tab.) If you don't have the interfaces modeled you should look at its snmpd.conf file to be sure that Zenoss has access to the full MIB tree.

The ip address from which the syslog message is coming from (and is shown in the event console in the device column) is on the os page of the device. One thing I noticed in checking that interface (eth5 in this case) is that it's operational status is shown as being down on the os/eth5 page, despite the graphs showing traffic for the interface.

I'm not sure why it would be marked down, when I look at the IF-MIB entries for that interface, it says it's up. And it is up of course, since that's the ip address I use to connect to the machine with ssh, collect SNMP data, etc.
