I do not like to see things pesimistic, I rather look to them from realistic point of view.
We (mexican company I owning with english partner) just launched a project for doctors here in mexico and negociating more than 14 month about hospital contract. One is sure - doctors here in mexico NEVER reveal information to their coleagues. They are not talking each other. They do not provide information about their patients to the hospital. They do not exchange any data which they must not.
Do you think that there is any chance that german patient in mexico will have an access to the system in Germany? Did you ever been longer time in country like mexico? Just try to be rather practical than idealistic. Real world is somewhere else.
As I commented several time ago there is and always will be question - how much cost me this or that? And honestly billing module I saw from ecomscience is less usefull than excel spread sheet.
If care2x is to be marketable there is need for serious development on billing module which is totally integrated with care2x system. We started on integration with SQL-ledger which seems to be most complex and customizable accounting package.
There is better if one tool do less and well than everything and nothing well. I would say care2x is amazing project, but without proper planning and GOOD marketing cannot become comercially succesfull nor widely use.

There is a need if you like to have a conection from a hospital in Mexico to a hospital in Germany. Independant. The German Patient is in Mexico. He can give access to his data in Germany. Than treated there, coming back and the German family doctor knows what happended there if the patient give access. Thats only an example of transcontinental data transfer. For health systems and insurances the effective costs go down. It can be used as a possibility of billing transfer or other things.
And the main important. The decition of the data use. All is in the hands of the patient.
