By default the search page is installed in /opt/www/htdocs/htdig. The file name is search.html. If you used --with-search-dir when you ran configure, then the search page is in the directory you specified with that option. By looking at search.html, it should be clear what you need to do in order to create a customized search page. The content of the 'form' element is the important part.
