You can write your own MAPI services provider,  Outlook can use
it to interact with whatever server you want. That's what HP
did with OpenMail (you need to install their Outlook OM MAPI
services to use it with OpenMail, other then that nothing special).

Of course, you'll probably need a mail server with some extra
functionality besides delivering mail (MIME conversions, lots
of server-side processing, scheduling - but that's easy,
Exchange saves that inside a special public folder).

Never used any fax software with Exchange, but all things
considered, it's still a good product. It's commercial, these
things cost money. Check out the price tag on Lotus Domino(/notes) or
Netscape Enterprise Server. Silly quirks aside.
