Mine's much the same as Paul - *ist-D, 16-45, 50-200 and AF330FTZ flash.  
Battery charger
and one spare set of batteries, lens brush, and that's it.  For my most recent 
trip, my
wife also took her Optio, which would have served as a backup if needed.
In film days I carried MZ-S, Sigma 18-35, FA28-105, and the same flash, plus 
some film ,
on the assumption I could buy more in most places.  Backup was a MZ-5.

For the film trip, we were limited to a 15kg backpack each, but could also 
carry one other
bag, so I had my Lowepro with the camera kit slung in front.
