In the matter of multi-factor authentication, discussions are increasingly quoting a fourth factor: "WHERE you are". (Does the local electricity company REALLY want to accept an administrative log-in from Paraguay?) It seems reasonable to (at least try to) incorporate this - whenever it is practicable. For me at least, a country-based ACL, recompiled every week, is (almost) a fit-and-forget solution.
