It may be worth asking Quanta - as they had a reprint of the book, there
must have been an original file somewhere...
Presuming (of course) that Jan Jones gives her permission for the book
to be made freely available.

more significant figures?
Jan Jones, on page 115, says that
OPEN#3,SCR_137x100 will give a square window. I found that a rounded 135
seemed more accurate.
