or, the speech in safe mode add on that is available on my grab bag site.
and either a set of USB sound card and speakers or standard head phones; or
(best actually) a set of USB headphones since there considered a external
sound card and have there own drivers that windows will find and install in
98% of cases

you will probably want to learn the commands for a lighter weight screen
reader like NVDA as well, since it seems easier to use in the safe mode
speech operations, but you can set up a cheat sheet and go from it while
using the safe mode operations as long as you know the few basic commands to
command line/run command  start the screen reader, for getting to the
desktop,  starting the sheet (I'd just put a shortcut to it on the desktop
to speed things up), changing to a text editor window, and to move in a text
editor reading window

that will give you speech in windows 7, 8, and it has been said XP as well,
though I have not tried the second or last ones yet myself

not normally

but if you get the safe mode speech package off my site it can be made to
work or at least NVDA will I can't remember if jaws does with this package,
but NVDA is fairly good. 
you will also need either a external USB sound card and headphones or
speakers, or a pair of USB headphones

all should be explained in the safe mode speech package.

myself after losing sight. I was under the impression that JAWS should load
in safe mode too if the computer has it installed because JAWS loads as a
service.

I use to have a system registry addition which allowed speech to start in
Windows 7 at Safe Mode. If you ask around to the CAVI folks or go up to
their web resources it might be up there.
