directly from FrameMaker (is supposed to be improved in the
last versions) or use Mif2Go ($ 295, http://www.omsys.com).

(with anchored frames/tables) from the reference page.
Try out Reficons from Cudspan whether it does what you want:

Your markers appear in front of the chapter titles
but not in the TOC, don't they?
When a TOC has hyperlinks to headings, hyperlink markers
are created only in the TOC. Therefore the markers in
front of the chapter titles must be something different.
