Every router is different. You will need to add a port forward for port
5900 to the internal IP address of the server (also port 5800 of you
want to use the java viewer). Once that has happened, you will be able
to remotely connect using the external IP address that your ISP has

Then, from your friends house, either point the VNC viewer to that IP

The problem you're seeing is that the programs you're using assume that
you'll only run one copy per-user - they aren't capable of running as the
same user but on different desktops. This is a long-standing issue with
many Gnome applications, if I remember correctly.
