What email program are you using?  I've had kind of the same problem 
with Eudora.  If you are using Eudora, have you tried backing up 
everything in your Eudora folders, then uninstalling and 
reinstalling? Only change the data dictionary when you are installing 
- not through Eudora afterwards (leave that attachment directory 
blank if you go in under tools, options, and attachments.

I think my problem has something to do with the index file in the 
attachments directory, because when I use Windows Explorer to copy 
the attachments from backup into the attachments folder, that's when 
I have problems with Eudora not wanting to download and open 
attachments correctly.
