commit: Fix segv on zoom with header. Other fixes.

Fix segv on zoom with Header, Bullets for bullet lists, cursor blinks with
if doc has a header, other header bugs fixed.

Still haven't fixed the field on start on line bug though. I'll try to
nail that next.
