In which organization you are working? Broadly, it seems your problem has two aspects: first, you are not able to fulfill the job assignments provided to you due to your physical problem. secondly, you are not able to deal with people around you. Means, you are having problem communicating with them or you are shying/hesitant to tell exactly the MAGNITUDE OF problem you have. Every organization has different layers at different level.
