One or two domains (empty root plus a production domain). Either works.

I'd probably do one site per location that has a domain controller. You've
left out the sizes for the locations, so I can't really comment on whether
or not to put a DC and therefore a GC in each location. Generally, I don't
put DCs in sites with less than 30-40 users.

A more general piece of information is that in both a single domain, or an
empty root with single domain structure, there is no appreciable overhead
for making all[1] domain controllers also function as global catalogs.

[1] Except the Infrastructure Master FSMO role, of course, although
