  Tsunami DVD Author sounds like just the ticket for
  you.
  It can do plain old ordinary menus, and it can do
  fancier stuff like motion menus with animated
  thumbnails for the chapters, and things like an audio
  track for them too. I think it can do video
  backgrounds but I haven't bothered to try that yet,
  nor have I tried making my own buttons or thumbnail
  frames since it comes with a decent selection of them.

  If you want really simple menus, Nero Burning ROM can
  slap together real simple menus. Just start a DVD
  Video progect and feed it the DVD format MPEG2 file of
  your choice with one audio stream muxed with it. (The
  only thing I use Nero for is burning discs. The rest
  of their bloatware download is pure fluff. I wish
  they'd offer a "Burning Only" package so I don't have
  to waste time and space and money on what I don't need!
