There are bulkhead feedthroughs that fit a D shaped hole, and the SMA threads start on the outside, but you are looking for 16 mm of thread outside the box. That’s a lot.
This for example only has 14 mm of thread, and only 6 mm of that is the SMA thread. http://www.l-com.com/multimedia/eng_drawings/BA21.pdf It doesn’t sound like the mating connector needs anything close to 16 mm of thread. Why do you need so much? Or when you said the plate is nearly 4 mm thick, did you mean 14 mm? Maybe that’s it. 4 mm isn’t really very thick, like 5/32 inch. Not enough to consider counterboring. It's easy if you have a milling machine of course, but I wonder if there's some kind of drill bit that would make a countersink just big enough for the flange on your bulkhead connector.
