There are two separate issues that I'm aware of which would prevent the 450i from getting sync assuming good pulses coming out of the injector...
The first is the power polarity swap we described. It's going to prevent a 450i from getting sync ever. The second is the issue with the transition noise sensitivity. In general, if the sync pulse has some noise (bounce, etc.) at the transition periods, then the internal digital circuitry will see multiple pulses, which causes the radio to ignore all of them. There is a hidden command which permits the reception of a sync pulse with the multiple pulses in this case. Occasionally this will be intermittent based on external variables (changing high power RF environment, etc). There is of course the issue that the sync pulse isn't present at all. Assuming an out of the box powerinjector+sync, and a well-positioned GPS device, this isn't likely, although it could explain the 15 minute delay since it takes that long sometimes for the GPS to get the initial lock at a new location... but generally if you're getting one and only one yellow led blink a second on the injector, this won't be the issue.
