[AFMUG] Phone line passthrough for in-wall AP

Looking at installing a few hundred in wall APs somewhere where they are
still using the phone lines.  There is currently Cat5 lines, one phone, one
data, run to a single low voltage box.  Rather then having to cut and
install a new low voltage frame, does anyone know of something like an
extension ring with an RJ11 port on the side?  Seems simple and I feel like
I have seen them before but can't seem to find anything like it.
