Not clear, in wall AP or in wall ATA? I would think a wall plate with both RJ45 and RJ11 on it.

There is currently Cat5 lines, one phone, one data, run to a single low voltage box. Rather then having to cut and install a new low voltage frame, does anyone know of something like an extension ring with an RJ11 port on the side? Seems simple and I feel like I have seen them before but can't seem to find anything like it.
