The only thing of note in the log is:

Not sure what this means. The /proc/asound/card1/stream* files don't 
seem to contain any errors, content I assume is OK... or not??

Where is alsa-kernel/usb/usbmixer_maps.c?  I have kernel sources and 
all alsa-related files installed with my SuSe 8.1 distribution, but 
can't find this file.
