Why do you think so many places have some sort of "log on and evaluate us" offers? (Yeah, they're also collecting email addresses, but that's a minor point.)
It's hard to get people to give good evaluations of anything. For every person who complains to management, fifty will complain to their friends, or on their Facebook page or wherever. To get good feedback you're going to need to be proactive somehow.
