Oh you software people always think everything is a software problem! SD Card failures are sometimes a mechanical problem.
If you pop out the SD card and push it back in again, does that fix it? Did for me! Until after the 20th time I got tired of the exercise, which makes taking pictures really slow... The SD Card specs allow some tolerance for different thicknesses, and it is tough to test the socket with all of them. And the contacts can corrode and the springs can get weak. I had this problem a lot but I fixed it for the last week, knock on wood. I simply stuck a piece of sticky label (intended for a video tape) to the size of the back of the SD card away from the metal contacts. These labels have strong glue and are unlikely to come off inside the phone or jam the card in permanently. That presses the metal contacts a bit more, and now the camera works every time!
