So is the issue that you need to use Japanese with mutt in console or in
X?  In X, it's quite easy.   In console, you might have luck reading it,
though I don't know about writing it, using kon.

You shouldn't need a unicode fix for mutt, you simply have to set your
LC_CTYPE before writing the letter.  For instance, (assuming you use
scim-anthy for input)

XMODIFIERS='@im=SCIM' LC_CTYPE=en_US.utf8 mlterm -e mutt

If your friend is using MS, it's probably ShiftJIS.  If he's using Mac
OSX it's either Shift JIS, or, in later versions utf-8.  (BTW, with the
LC_CTYPE I mention above, I'm not sure about upper and lower case,
right now, I'm writing from FreeBSD but the link I gave above tells you
how to check if your LC_CTYPE should be UTF-8, utf8, utf-8 or whatever. 

You need an xterm capable of handling it, rxvt-unicode or mlterm.
Actually, even with xterm you can use uxterm.  

UTF-8 in mutt is fairly trivial, and, in an xterminal at least,
shouldn't require anything more than making sure you have your locales
(also covered in that link, there's an ArchLinux section) and
scim-anthy.

Do you mean use vim, or input Japanese in web browsers.  The second one
is also fairly trivial.  You have to make sure you start it with the
LC_CTYPE or have QT_IM_MODULE or GTK_IM_MODULE set.
