I'd say check what is being sent to the shr:consolidatedlist, then look at your submit and modify filters (on my 6.3 install they are called shrc:shr-modifyconsolidated1&2, and shrc:shr-submitconsolidated). Are you sending the full name or the Assignee Login Name for the Assigned To field?
