I ran into this before and after we combed through the AR Email Engine and subsequent process, we could not find anything different. We start a conversation with our Exchange Admin about possible Exchange issues and it ended up being a bug in Exchange 5.5 as listed in the KB entry below. Not sure what version of Exchange you use, but this may help you in your research.
