The amount of customization, timeline, number of end users, etc. With my company, there's 1 1/2 of us and a lot of heavy customization to the 5.5 ITSM, but it needs to be reliable, so it's taking us a while. They aren't in a rush though so it's cool. In such a competitive market, there is almost always someone who will do it faster, cheaper and with less people (doesn't mean that the quality of the end product will be better). Rick is correct though. If the customer wants it done in two weeks vs. two months or wants a vanilla install and config vs. a customized solution, the numbers and prices can change dramatically.
