Thank you Kelly. That's a much better answer than what BMC EIE Support was giving. Since the destination form that they were looking at only had three integer fields on it, that's all that they could see in that menu and that's what they expected me to see also. In their defense, they are otherwise helping a great deal.
As it turns out, there is a not-null unique column in my source table and using that for the Key field appears to have solved the duplicate record problem. Now it appears that the records aren't being updated correctly, so I may have to use the "...Only if Values Change" option after all.
