I contacted BMC regarding this issue over a year ago and they identified 
it as a defect around the GUID on the form -- there is a unique index on 
that field.  Their workaround was to remove the index.  They never 
identified the cause of the issue.

Workflow problem

Hi All

I am noticing an error message in the arerror.log file. The message is as 
follows.
 
The value(s) for this entry violate a unique index that has been defined 
for this form: NTE:SYS-Group NT Control entry NTG000000063532 fields: 
1000000205 179 (ARERR 382)
 
Ok I understand what the entry means, what I cannot find is, what is 
causing this to happen. Obviously some workflow/process is trying to 
create entries in this form. But what process or workflow? does anyone 
have any idea?
