Do not underestimate the power of a restart. :-) We had a similar issue, but it only affected people with a blank password in their User record. If I remember right, the Application or Midtier password was wrong but did not automatically fix the issue when we changed it. We had to restart Midtier in order to get the problem fully resolved.
