Restart the Mid-Tier server for a good measure.. According to the error message guide, 9215 is a general all-purpose error. Happens under multiple circumstances some of which include internal cache error, or web tier exception, or a system object or field being set to null.. Also make sure that you are using the right URL as earlier pointed out by Fredtick.. If you have saved the login page as favorites, make sure you have not saved the redirected page. In version 6.3 that could lead to an error (I do not recall the error number).

With Mid-Tier you specify a form to open and if you are not already logged the web server brings up the login page. Accessing the login page by itself does not help as the server does not know what to do after that.
