You need to take a look in the <itsminstalldirectory>\Logs directory. There should be some additional logs in there that have the output of the various external commands that are being run to import/update forms/data on the server. The return code below suggests error 392;
Error Number: 392 Error Type: Error Error Text: Field or VUI name must be unique for the form -- there is already a field or VUI using this name. So I suspect there's a failure importing an updated definition for one of the Change related forms due to some sort of naming conflict. Try looking in the BMCRemedyChangeManagement.log file first - but you may need to consult some others depending on exactly where the failure occurred.
