I've changed the convention of adding new FAQs to the top of the list to help
maintain the numbering:

The original reply to Dag was in August when it was FAQ #2, another
FAQ was added 2 Sept so its now FAQ #3
If the FAQ number appeared to be wrong it would be a good idea to
check the rest, it could be a typo.
I've searched all of the asciidoc FAQ page, and there is _no_
reference to http://sagehill.net/docbookxsl/PrintHeaders.html
The FAQ points to the parent TOC so that it covers the whole topic.
That TOC points to this page.
It would have been a lot more helpful if you had simply pointed to the
pages at http://sagehill.net/docbookxsl/ rather than making cryptic
postings about FAQ#2.
The FAQ points to *several* useful parts of the documentation. And
pointing to the FAQs in general reminds all people on the ML to read
them.

Indeed, but I have no idea where you expected anybody to find a link
to that page. It's certainly not pointed to by FAQ#2.
The FAQ is called "Sources of information on configuring DocBook
toolchains" and as I said above has links to the TOC that contains
that page.
