Lex, Asciidoctor does fully support underlined section titles.
We just don't put them in the quick reference because we believe strongly they are a not a good choice. I've heard many people complain that they don't like them, they are confusing (since you have to remember the symbol to level mapping) and they are actually deterring people from adopting AsciiDoc.
