I've seen some of the IVR solutions here and while they work, I tend to find 
them too aggressive for me.  I prefer a little bit of a different approach:

if a Caller*ID matches a blacklist, dump to voicemail.  If it matches a 
whitelist, dial the inside phones normally.  If it's empty, from tollfree or 
invalid, play SIT, wait a few seconds, then call the inside phones normally.  
Otherwise, call the inside normally.

Seems to work pretty well, and the SIT tone will help get me out of the caller 
databases since the equipment generally detects it and notes the number is 
not in service.
