My customer that still does large volume incoming and outgoing faxing uses a system I set up for them - Linux, Hylafax, 8 port Mainpine analog fax card into 8 analog lines. The nice thing with this was that the card supported speeds up to 38400 - a lot of other solutions can only handle 14400. This was entirely separate from their voice service. At its peak I was seeing something like 20000 pages per week - but the volume is moving away from faxes.

Another install we use VoIP instead of analogue lines and it's flawless also.

But outgoing is always a problem.
