Perhaps you should look at the progressinband parameter in the sip.conf? or
I guess in this case maybe in the users.conf.. it will send inband ringback
tone to the caller if perhaps the sip provider is establishing an audio path
even though the call is not yet answered..

First step in the debuging process should be to provide the asterisk logs
(from cli and/or the full one) for a "good" (with Astlinux 0.6.2) and a
"bad" call (with Astlinux 0.6.5 for example)
Do not forget to enable the debug and verbose as I know that by default them
are disabled in Astlinux.
Secondly - from the messages on this thread I did not understand exactly
what card do you use for the FXO port. Maybe I missed the information. Could
you give some details?
Basically, the ring which is initially heard by the calling party is
generated by his or your provider switch. From the initial post I understand
that you have tested with a mobile cell phone. Is the same behavior reported
if you will dial to the Astlinux box using a line from the same provider as
the one you use in the FXO line? I expect to have the same situation....but
who knows.
As I suppose that you are from Romania (from your name) - could you specify
exactly the providers you have for the PSTN line?
One workaround is to answer the call after 1-2 seconds (needed to retrieve
the caller id), play the ring tones for 1-3 seconds and after this to route
the call where you need. The drawback is that the call will be always
answered, meaning the calling party will pay even if the call will end with
a hangup event due to a routing problem).

I am told is my dial plan. What's interesting is that the same configs work
fine in 0.6.2 image that I am running right now.
I will do a 0.6.5 fresh install and see what happens.....

The problem is in your dialplan. Please send me your extensions.conf. This
is really an Asterisk issue though and not Astlinux specific.
Please post the solution to this as I'm curious about the fix.
