Let me clarify one thing as NonReality really said it the way I mean it. iTunes does what it is intended to do, but just like any Windows OS, it does things that one would never want it to do if one knew it were doing it. Never, never ask it or allow it to go search for album art or fix your files or tags! The way to prevent this is to protect ones library. The way I do it is by ripping all my CDs to FLAC into one directory called Music with subdirectories of Artists. At the same time as the FLAC rip, dBpowerAmp also rips at 320Kbps MP3 into a directory called MP3. If I only want selected songs or a song from a CD in MP3, I just rip to FLAC then come back later and convert the file to 320 MP3. SqueezeCenter is pointed at Music for its files and iTunes is pointed to MP3 as its files location. So iTunes never sees my safe and sound FLAC files. If iTunes hoses something, I delete the hosed files and convert to 320 MP3 again from the protected FLAC files. I never let iTunes do anything but put songs on my iPhone. I dont allow it to organize, search for things, fix, re-tag, replace album art, nothing. iTunes is terrible when it comes to album art. It just assumes that you have the latest and greatest pressing or remastering of the CD because after all one should have bought it just now from the iTunes store. I have over a 1000 CDs from the early 1980's and iTunes wants to change the album art on all of them to whatever re-release is currently available, no thanks!

A 24/96 file is a file that is of a higher resolution then ripped CD files that are at 16-bit/44.1KHz. The 24 is 24-bit and the 96 is the sampling frequency of 96KHz. It has nothing to do with Apple Lossless. 24/96 are HiRes files that are available for Download from some sites which one needs a Transporter to play at their native resolution.
