Using Dan Lewis's latest iteration of this chapter, I fixed some
problem lists, re-anchored some picture frames so they no longer
wander away from where they should be, amended the info on extensions
that I had written at the end of the chapter, and did some minor
copyediting. I have submitted it for further review and testing of the
instructions.

Dan did a good job of fixing most of the problem lists, but some lists
continued to refuse to restart numbering when the file was re-opened.
Rather than continue spending time trying to solve the problem, I
changed some of the a,b,c lists to bullet lists, which seem to be
working fine. So there is a bit of inconsistency in the presentation
of lists in this chapter, but I don't think it affects usability and,
indeed, I suspect  most readers won't even notice.

BTW, I have not taken the time to try to extract a reproducible sample
of the problem and file an issue. I think it is related to this
chapter's a long and complex history of multi-level lists interspersed
with figures and tables (tips, notes). If anyone has seen a similar
problem (refusal of lists or sublists to retain a "restart numbering"
command), of if you are aware of an issue already filed about this,
please let me know.
