Because in that case I don't mark my changes and it won't be immediately visible to the reviewer what I changed. Unless you have a clever diff tool, in which case I'd ask for a pointer to this tool.
Lot's of IDEs (and all serious unix editors) can do stuff like code coloring on diffs; I believe (even) an OSS tool like Jedit can do a split pane view with changes highlighted. I use JBuilder (saving up for IDEA though, which I'm trialling ;).
Basically you apply the patch locally, and then you can have your local revision and the cvs version side-by-side, with changes highlighted. Works really well.
