The reading is still map based.
this means some kind of scale must be passed to the class.
say your map is 1000 on width and your model is twice as large, a factorW of 2 will correct the scale issue for you.
same rule applies for factorH and factorD.

As about the "bumpy" fx, depending on your model, you can pass an int in your constructor
new ElevationReader(2);
It would in this case force the class to return you an average of the 2 last readed levels. As a result the movement are much smoother.

also, if i scale the original extrusion how does away recognize that
now is bigger,

lets say i scale to 50:

i wast thinking of make it this way in the render

elevationreader.getLevel(camera.x/50, -camera.z/50,0));

but im afraid that maybe i have problems with the offset, is this the
correct way of doing it, if not what can i do?
