Andrew, There is a book called *Armorial Lusitano *which gives the origins of the Portuguese names, not necessaryily Azorean. The book is all in Portuguese but has the coats of arms and a brief history of the names. I bought the book several years ago from a book dealer in Lisbon. This is what is on the bottom of the box that holds the book:
