everyone I've ever seen has been in the form:
http://{encoded domain}/plaint/text/path.cgi
Here are some examples that popped in a quick search of old spam.
But none of those had encoded domains, which is what the original
post was about.

I think you meant %25hh. Apple recently changed their iTMS http URL
processing, now redirecting via a Location: header containing such
double-encoded characters. I had to update my http-to-itms URL
resolver script. :-)
