Ok...apparently my previous wpa_supplicant config was interfering. Just
for kicks I decided to edit wpa_supplicant.conf, and simply
commented-out everything that I had in there to make ndiswrapper work
with my WPA-PSK settings (I thought I had tried this before with no
results). Voila (after several attempts), it works...I can register an
address with the DHCP on the router. However, it will only maintain a
connection for about ten seconds, then it dies without any error
messages at all (e.g., I can load a web page, maybe do one or two google
searches, then it stops). If I unplug the card, then plug it back in,
I'll get my connection back for another few seconds. So what should I
look for next? Below are the new messages I'm getting...nothing looks
like an obvious error...the messages just stop after the dhclient script
finishes.

are you trying to use this driver as an access point ? I don't know if
it's supported but i'd so it's not.
For associating with my ap i do:
iwconfig eth1 essid <myessid> channel <mychan> [rate 11M]
I'm currently using an old version of the driver, with a 2.6.15 kernel,
but it works great.
