For those keeping track, this particular issue prevented activation of
all users on the system, existing unactivated and new users.
Deleting/recreating did not help, no errors in event logs or anything
particularly useful in the BES server logs. Same for restarting BES
services on server. The users on the server saw no problems with any
service at all the entire time, ie, everything else was working fine.
Miraculously, a full server reboot and (probably more importantly) a
RAM increase resulted in multiple successful activations! The server
was almost out of RAM, but I have never seen that affect an activation
before.
Anyone else seen that as a solution before? Thanks!

Did you try restarting BlackBerry Synchronization Service and
BlackBerry Policy Service?
Usually, those services get stuck from time to time.
After restarting the above mentioned services, reset the activation process.
