of the cache contents in /var/lib/squeezeboxserver/cache including
library.db and restarted LMS and the problem continued to happen. As per
your instructions I have stopped LMS and deleted library.db (again) and
restarted LMS. It is now doing it's third rescan for no apparent reason.

Please disable all third party plugins and try again. There are at least two (trackstat, dynamic playlist) which are causing database locking errors. Leave them disabled, shut down LMS, delete library.db, make sure there is no other instance of LMS or its scanner running. Restart.

If this does work as expected, re-enable one plugin after the other, doing rescans in between.
