Your log is confusing - if the log is shown playing a single stream then there is something seriously wrong as it goes through the process twice in which case I suggest reinstall LMS.
Please make sure you only included logs for a single session and note the time when you do the test so you do not include extraneous logs (or better still stop LMS, delete log files and just do one test). The log file says you have disabled some filetypes.
