Most parents get their kid's helmets from places like Toys R Us and
Wal-mart.  The clerks there don't have a clue how to fit a helmet. 

When 80% of folks don't fit their helmet properly the problem
ultimately lies with the manufacturer. I read about a 12 year old girl
who designed a helmet that had lights flashing when the helmet didn't
fit properly and was looking to sell her idea to a manufacturer.  There
would no doubt be some kids who would probably maladjust their helmets
just to see the flashing lights.  But if a 12 year old can come up with
an innovative way of improving helmet fit you'd think that Bells'
mechanical engineers could come up with some solutions.

Another issue here is what I consider to be the
responsibiliy of helmet retailers to make certain that
a customer making a helmet purchase is getting the
right size and best fit possible. All to often
ill-fittting bikes and helemts are sold to customers
who do not, themselves, have the knowledge to
determine what fits them the best. This doesn't do
much for less experienced people's attitudes towards
bicycing and, as has been pointed out, can be
dangerous as well.
