Another steep one is South Gammon Road, from Elver park to Raymond road.
I can't get Veloroutes to give me an accurate answer, but its at least 15%.

Don't know whether it's the steepest grade in the city, but Old
Sauk Road between Old Middleton and Ozark Trail is about 12%. And I
curse it every day.
